Notes on Altera State Machine Stuff

- In the INSTRPRC state, a rerun acknowledge is not decoded completely
  (only ack0 and ack1 are used).

- In the RRNWT1 state, the polarity of Bus Grant which is tested is
  inverted.


- Note: Dave has changed one of the state machines in the vti source
   files but not in the altera files.  The affected file name is
   /im/cam8/interface/sctlfsm.sm
